Psychotherapy - Health Plan of San Mateo Insurance
Prior to starting services, you must request a referral to The Curiosity Center from Health Plan of San Mateo. Their referral team can be reached at 650-616-2557.

Psychotherapy - Mutual of Omaha, EAP
Prior to beginning services, The Curiosity Center must receive a referral from Mutual of Omaha. Please speak to your case manager to request a referral.
Mutual of Omaha offers coverage of a limited number of sessions, typically 3-6. For best utilization of time, it is important that you complete all paperwork prior to the first session, If paperwork is not complete prior to first session, you and your therapist will complete the paperwork together during that time.
